Observation

At the time of the unannounced onsite Monitor Visit, there were two blinds with strings, capable of forming a loop, present in the living room. At the time of the unannounced onsite Monitor Visit, there were multiple wires belonging to the tv, tower fan, and power strip that were capable of forming a loop, and eaccessible to children present. At the time of the unannounced onsite Monitor Visit, there were multiple plastic bags in the unlocked bathroom cabinet (under the sink) that were accessible to children present. At the time of the unannounced onsite Monitor Visit, there was a light switch in the dining room that had a screw sticking out, creating a cut/abrasion/pucture risk. At the time of the unannounced onsite Monitor Visit, the washer/dryer were accessible to chilren present and did not have a strap/lock to prevent entrapment. The shed in the back yard was not made inaccessible to children and posed an entrapment hazard. At the time of the unannounced onsite Monitor Visit, the stove exhaust vent was rusting and had paint flaking, which fell to the stove below. At the time of the unannounced onsite Monitor Visit, the bathroom outlet was not tamper resistant/and was not covered with a blank face plate. At the time of the unannounced onsite Monitor Visit, there was a power strip under the tv that was accessible to children that were present.
